Advance Care Planning (ACP)
Plan Ahead for Your Future Care
Advance Care Planning, or ACP, helps you think about and share your wishes for your future health and personal care.
It includes your personal values, beliefs and healthcare preferences. These can be shared with your loved ones and healthcare team so they can better understand what matters to you.

This logo reflects the heart and mission of Santunan Emas. It symbolises the trust to care for seniors with deep compassion, and it highlights the value of unity and appreciation for them with full respect and dignity.
Overall, this logo serves as a reminder that in Islam, preserving and honouring seniors is not merely a kindness, but an act of faith amanah that continues the love and care they have poured out throughout the generations.

Santunan Emas
Santunan Emas is a weekly initiative by Masjid Al-Muttaqin, created to serve the elderly community with care, connection, and continuous learning.
Through meaningful sessions that combine Islamic guidance, active ageing, and social interaction, we aim to support seniors in living their golden years with purpose, dignity, and faith.

How Does It Help?
Support may include:
Helps you think about the care and medical treatment you would prefer.
Allows you to share your wishes with your family, caregivers and healthcare team.

Helps ensure that your future care reflects your personal values and preferences.
Helps your loved ones make decisions if you are unable to speak or decide for yourself in the future.
Can reduce stress for your family when important care decisions need to be made.

How to Get Help
You can start your ACP by:
Thinking about the type of care and treatment that matters to you.
Talking to someone you trust about your wishes.
Using the myACP online service if you meet the requirements.
Making an appointment with a trained ACP facilitator for guidance.
Speaking with your doctor or healthcare team if you have an existing health condition or chronic illness.
